State W-4 Form List

Purpose

Employees complete a federal W-4 for federal withholding, but most states have their own withholding certificate. Use this reference to find the correct state form when completing an employee's State Tax Withholding, during Setting Up Employee Payroll or a later update. The State Tax Withholding tab in UZIO asks the questions from the applicable state form.

Who needs this: Admins entering state withholding on an employee's behalf, and employees completing their own forms.

Form numbers are as listed in September 2026. States revise their forms. When in doubt, search the state revenue department's site for the form number below rather than relying on a saved copy.

States with no state W-4

Alaska, Florida, Nevada, New Hampshire, South Dakota, Tennessee, Texas, Washington, Wyoming have no state income tax withholding form. Pennsylvania has no general employee withholding certificate (flat tax), but see the reciprocity-exemption note below.

Note: New Mexico, North Dakota, and Utah use the federal Form W-4 itself; they have no separate state form. Delaware has its own state-specific form (Form DE W-4), not a variant of the federal form.

Note — reciprocity exemption forms: A few states also have a separate form for employees claiming an exemption under a reciprocal-state agreement, as opposed to standard withholding elections: - Pennsylvania (Form REV-419, Employee's Nonwithholding Application Certificate): for employees who live in Indiana, Maryland, New Jersey, Ohio, Virginia, or West Virginia and work in PA under PA's reciprocal agreements with those states. - Arkansas (Form AR4EC(TX)): a Texarkana-specific variant of the standard AR4EC for employees who reside on the Texas side of Texarkana and are exempt from Arkansas withholding. These are not a state's general withholding certificate. Use the state's standard form (above) unless the employee's specific reciprocity situation applies.
