Enrolling in Benefits: Step-by-Step

Purpose

How to choose your plans and get your elections submitted, whether you are a new hire with an enrollment window or an existing employee in open enrollment.

Before you start

Have these ready:

  • Dependents' details: full name, date of birth and Social Security number for anyone you intend to cover.
  • Your doctors' names. Some carriers ask for a primary care physician for each person covered.
  • Details of any other insurance you or your dependents hold, which carriers use to coordinate claims.

Enrollment is done in the employee web portal.

The wizard works through these steps. The exact entry screen you see, and how your deadline is presented, depends on whether you are enrolling as a new hire or in open enrollment.

  1. Start enrollment from your Benefits area once your enrollment window is open.
  2. Confirm who is covered. Review the people already on your record and add any dependents you intend to cover.
  3. Choose your plans for each category, such as medical, dental and vision. You have two ways in:
    • Create Your Own Health Profile, a guided questionnaire that asks about your expected care and then recommends plans that fit.
    • Show Me Medical Plans, which lists everything your employer offers.

Each plan shows your cost alongside what your employer contributes.

  1. Continue Enrollment to check out. At checkout you:
    • Verify everyone's details. These details go to the insurance carriers, so a typo here becomes a problem at the carrier.
    • Add doctor details where the plan asks for them.
    • Declare any other insurance policies you or your dependents hold.
    • Accept the Terms and Conditions and sign with your first and last name.
  2. Submit Application. You see a confirmation screen, and your elections appear on your Benefits Dashboard.

Note: If you cannot complete enrollment yourself, your employer can complete it on your behalf. Self-service is the normal route, so ask your HR team only if something is blocking you.

After you submit

  • Your elections go to the carriers through your employer's enrollment process. Carriers issue insurance cards directly and send them to you. UZIO does not produce or store them.
  • Paycheck deductions update to match what you elected. See Understanding Your Pay Stub.
  • Your elections are fixed until the next open enrollment, with one exception: a qualifying life event such as marriage, a birth or losing other coverage opens a special enrollment period. See Reporting a Life Status Change.

Warning: If you miss your enrollment window, you cannot elect coverage until the next open enrollment unless you have a qualifying life event. Note the deadline the day your window opens.

On the mobile app

Enrollment is not available in the UZIO mobile app. Use the employee web portal, on a computer or in your phone's browser. The app may show your current benefits, but it cannot enroll you or change an election. See Using the UZIO Mobile App.

Which plans are offered, what they cost, when your window opens and closes, and whether a particular dependent is eligible are all set by your employer and its carriers. Contact your company's HR or benefits team, or your company's broker, for anything about the plans themselves.
